The Home Care Packages program provides older people who want to stay at home with access to a range of ongoing personal services, support services and clinical care that help them with their day-to-day activities. All Home Care Packages are delivered on a Consumer Directed Care (CDC) basis, which means that the consumer has choice over what package of services they receive within their allocated funds.
A government-subsidised Home Care Package provides you with all the support and services you need – domestic assistance, personal care, nutritional assistance, transport and shopping help, allied health services, care related equipment and care related home modifications can all be funded under a Home Care Package.
Levels of Home Care Packages
There are different levels of Home Care Packages that are designed for differing circumstances.
Level 1 Home Care Package – Basic assistance designed to give you a little extra support to help stay independent. This HCP level might include one or two visits a week from a care worker.
Level 2 Home Care Package – Low-level assistance that gives you the extra support you need to stay independent. This HCP level might include two or three
visits a week from a care worker.
Level 3 Home Care Package – Home care services for seniors who need more support to stay living at home. This HCP level might include visits from a care worker most days of the week and can include nursing visits.
Level 4 Home Care Package – High-level home care suitable for people with high-level or complex nursing needs. This HCP level might include visits every day from a care worker or a nurse.
